    Causes of Derailing Accidents of Inclined Shaft Trip Lifting in Coal Mine Based on Behavioral Safety

    • 摘要: 为了减少导致煤矿斜井跑车事故的人员不安全动作,基于事故致因“2-4”模型,利用统计分析和归类的方法对煤矿斜井跑车事故进行研究。研究结果表明:导致人员伤亡的煤矿斜井跑车事故的发生需同时满足3个条件:车辆脱离了机械控制;跑车防护设施失效;人员处于危险区域。导致车辆脱离控制非常关键的不安全动作包括:发现钢丝绳磨损严重仍未停止提升作业、使用无闭锁装置的连接插销、未将矿车插销插到底、未及时处理钢丝绳断丝超限的隐患。防护设施失效最主要的不安全动作:未处理跑车防护装置失效的隐患、串车提升未加挂保险绳、未安装跑车防护装置。人员处于危险区域导致的跑车伤害事故占事故总数的74.55%,包括2类动作:违章乘车和危险地点候车、行走、作业。72.73%的斜井跑车事故涉及共性不安全动作,其中违章乘车和危险地点候车、行走这2类动作发生的次数占共性不安全动作总次数的80.39%,是共性不安全动作培训的重点。

       

      Abstract: In order to reduce unsafe acts of personnel in derailing accidents of inclined shaft trip lifting in coal mine, by applying the accident causation “2-4” model, the derailing accidents of inclined shaft trip lifting were studied by statistical analysis and classification. The results showed that derailing accidents resulting in casualties required to satisfy three conditions: vehicles out of the mechanical control, failure of the protective facilities, persons in the danger zone. The most critical unsafe acts causing the vehicle to spin out of control including: do not stop lifting work due to the serious wear of the wire rope; use connecting pin without locking device; do not insert the tube plug in the end; fail to deal with hidden trouble of broken wires in wire ropes overrun. The main unsafe acts causing the failure of the protective facilities including: do not treat the hidden danger of protecitve device for derailing accidents; do not attach safety rope to the trip hoisting; do not install protective devices. The derailing injuries caused by people in dangerous areas accounted for 74.55% of the total accidents, including two types of actions: illegal driving and waiting in dangerous places, walking and operation. 72.73% of the accidents of inclined shaft trip lifting involved the common unsafe acts, among which the number of illegal driving, waiting for a car in a dangerous place and walking accounted for 80.39% of the total number of the common unsafe acts, which was the focus of the training on the common unsafe acts.
